Billy Joel, the 77-year-old piano legend, has delivered a sobering update on his health, revealing he may never perform on stage again. According to [1], the singer said he’s currently unable to drive or play live concerts, citing a 2025 diagnosis of normal pressure hydrocephalus—a condition where fluid builds up in the brain.

In an interview with The Hollywood Reporter published Oct. 15, Joel explained that each performance feels like a concussion to his brain, due to the intense sensory overload of lights, sound, audience energy, and stage stress [2]. "Every time you do a show, it’s like you’re getting a concussion," he told reporters.

Joel canceled a string of concerts after his diagnosis and underwent surgery in September to implant a shunt, a device meant to drain excess fluid from the brain [2]. Initially hesitant, he changed his mind after learning the shunt could improve balance and memory. He confirmed the procedure was successful and said he’s already noticed improvements in brain function.

Still, he remains uncertain about his future on stage. "I don’t know," he said bluntly when asked if he’ll tour again [2]. "They have to test me further to see if I can go back and do performing again, which I would like to be able to do."

Joel also downplayed fears, telling fans on SiriusXM’s The Billy Joel Channel that the condition isn’t as dire as it sounds. He acknowledged losing some hearing and balance but said he’s okay and doesn’t want fans to worry [2].

The claim that the shunt improved memory and balance is reported by one source only [2] and has not been confirmed by independent medical verification.
